What Is HeartStrong Community
HeartStrong Community is a 16-week virtual chronic disease management program for adults living with uncontrolled or poorly managed hypertension, and for those seeking to manage other chronic conditions while in community. The program is gender-inclusive and designed to meet participants where they are, regardless of background or ability to pay.
Small groups create a community close enough to notice when someone disappears. Every participant is supported by a clinician or a multidisciplinary virtual clinical team, depending on the program tier chosen, throughout the full 16 weeks, not just at the beginning.
This is not a one-size-fits-all health program. It is a sustained, expert-supported, peer-connected experience built specifically around your life and your health.
Who Can Participate
HeartStrong Community is designed for adults who are ready to go deeper on their own health management. You do not need to be in crisis. You need to be ready to do the deeper work of sustained self-management, with expert support behind you. To participate, you will need:
A Hypertension Diagnosis
A current diagnosis of hypertension or elevated blood pressure, or another chronic condition such as type 2 diabetes, obesity, or cardiovascular disease. You do not need to be in a crisis. You need to be ready to do the deeper work of sustained self-management.
A Blood Pressure Monitor
Your own blood pressure monitor, or the willingness to obtain one. We provide monitors to participants who need them at no cost. Consistent home monitoring is central to the HeartStrong Community experience and one of the most powerful things you can do for your cardiovascular health.
A Device and Commitment
A smartphone, tablet, or computer with internet access for virtual sessions, and a genuine commitment to 16 weeks of participation. HeartStrong Community is not a passive program. It asks something of you, and it gives you something real in return.
Your Clinical Team
Every HeartStrong Community participant is supported by a multidisciplinary virtual clinical team throughout all 16 weeks. This is not a single provider relationship. It is a team of specialists working together around your health.

The HeartStrong Collective
At the center of the HeartStrong Community experience is The HeartStrong Collective, a structured learning community modeled after a Community of Practice . Unlike a traditional support group, the Collective is expert-led and peer-interest driven, designed to build lasting health literacy, clinical understanding, and self-management capacity that extends well beyond the 16-week program.
Session topics include sleep and its relationship to hypertension, the DASH approach to nutrition, medication adherence, accessible movement, and heart-healthy cooking. Faculty and clinicians from IAWH's partner institutions lead sessions alongside community health workers who keep the conversation grounded in real life.
Enrollment
HeartStrong Community enrolls in small cohorts. Space is intentionally limited to keep the community close and the clinical support personal.

On the Horizon
IAWH Health is actively working to extend the HeartStrong model to reach women who face the greatest barriers to care. Two new programs are in development.
In Development
An adaptation of the HeartStrong Community model for women in rural and geographically isolated communities across Virginia. The program's fully virtual design removes the transportation and access barriers that prevent rural residents from engaging with traditional chronic disease programs. IAWH is conducting listening sessions in rural Virginia communities now to inform the program's design.
In Development
A program developed specifically for women over-the-road commercial truck drivers, one of the most underserved and medically isolated occupational groups in the country. Long hours, irregular schedules, limited access to care, and high cardiovascular risk make this population a priority. HeartStrong Road is being designed around the realities of life on the road.
